Some people find it useful to organize their thoughts prior to the first therapy session. Here are some points to think about.
 
  1. What events led to your decision to start therapy?
  2. When did the difficult problem begin?
  3. Do you get adequate, restful sleep most nights?
  4. How is this situation impacting your:
      Emotions and thoughts?
      Physical health and well-being?
      Relationships?
 
Also:
Please note on the Client Information Sheet the name and dosage of your medications and the name of the prescribing physician.
